BOOKING POLICY:

For the Issuance of Certification and/or Authentication of Eligibility, book through the CSC Regional Office tab located below.

For the Application for Grant of Eligibilities under Special Laws and CSC Issuances (SPELS), book through the CSC Field Office tab located below.

Ensure that the requirements for your transaction are complete and correct before heading to the Regional Office or Field Office. Your requirements are subject for verification by our Action Officer. Incomplete requirements or inaccurate data provided may be grounds for disapproval of your transaction.

All clients are required to arrive on their scheduled date and time. Otherwise, their appointment slot will be forfeited. No consideration will be given as transactions of other clients are also on queue and slots are fully booked. Clients coming from distant places are advised to travel early to be able to arrive on time for their appointment.

Invalid names will be deleted without prior notice.

Multiple bookings will be deleted without prior notice. Only the latest booking will be retained.

Only the request for certification and/or authentication of eligibility (or eligibilities) of ONE ELIGIBLE will be accepted per schedule.
(All transactions related to your eligibility will be accommodated in one booking schedule.)

If your appointment falls on a holiday or work suspension, your appointment shall be accommodated on the NEXT WORKING DAY.

For first time request for Issuance of Certificate of Eligibility, the eligible is required to claim the Certificate personally.
