Guide to Filling-out the ERRF and Declaration Form

 

 ERRF Guide

Please use this guide in filling-out the ERRF. Note that items labeled "critical" are needed, and your request will not be processed if these fields are not accomplished. 



1. Indicate date and time accomplished at the top right corner of the form.

2. Indicate number of copies requested for Certification / Authentication.

3. Indicate full name (critical). For married women, you need only to indicate "name used at the time of exam / issuance" if you are currently using your married name, and have taken the exam / conferred an eligibility when you were still single.

3. Indicate current address.

4. Indicate date and place of birth (critical). For place of birth, only indicate municipality and province or city of your place of birth (do NOT write the name of the hospital, barangay, lying-in clinic, etc.)

5. Indicate title of eligibility or exam, date and place of exam or issuance (critical, especially for eligibilities granted / conferred below 2010). Note: for SPELS grantees / applicants, indicate DATE OF ISSUANCE. This is provided in your COE. For new applicants, you can leave this blank.

6. ERRF must be ORIGINALLY SIGNED in the space provided, otherwise, we cannot process the request. For examinees that have changed their signature since the exam, you shall be required to redo your signature as signed in your Picture Seat Plan in the "Signature at the Time of Exam" field. This is part of the verification process, and your request cannot be processed if your signatures do not match.

7. Affix right thumbmark (critical).

8. Indicate contact number.

9. For those requesting via a representative (critical, for non first time requests only). Have the representative affix his/her printed name and signature in the space provided.

Declaration Form Guide

1. Fill out personal details as shown.

2. Check appropriate reason for not having your original COE (including first time requests).

3. Affix printed name and signature, date and right thumbmark. For those requesting via a representative: this form must be ORIGINALLY SIGNED by the eligible, otherwise, we cannot grant the request.

4. Fill out ID Cards presented section, with data indicated here consistent with the actual ID enclosed in your submission / request / application.

5. Incomplete or incorrectly filled-out forms shall not be processed.
