BOOKING POLICY:

1. This facility is for the appointment scheduling for the Issuance of Certification and/or Authentication of Eligibility of the CSC Regional Office V ONLY.

2. If you are applying for grant of eligibility such as Honor Graduate, Barangay Official, and the like, Send your application documents to the CSC RO V Field Office nearest you. Click here for the Directory of Field Offices and its email address.

3. Ensure that the requirements for your transaction are complete and correct before heading to the Regional Office in Rawis, Legazpi City. Your requirements are subject for verification by our Action Officer. Incomplete requirements or inaccurate data provided may be grounds for disapproval of your transaction.

4. All clients are required to arrive on their scheduled date and time. Otherwise, their appointment slot will be forfeited. No consideration will be given as transactions of other clients are also on queue and slots are fully booked. Clients coming from distant places are advised to travel early to be able to arrive on time for their appointment.

5. Requirements are indicated in the form.

6. Invalid names will be deleted without prior notice.

7. Multiple bookings will be deleted without prior notice. Only the latest booking will be retained.

8. Only the request for certification and/or authentication of eligibility (or eligibilities) of ONE ELIGIBLE will be accepted per schedule.

   (All transactions related to your eligibility will be accommodated in one booking schedule.)

9. If your appointment falls on a holiday or work suspension, your appointment shall be accommodated on the NEXT WORKING DAY.

10. STRICTLY NO APPOINTMENT, NO TRANSACTION.
