ALTPROC: Schedule of Fees and Payment Details

 

SCHEDULE OF FEES: 

1. Application for Grant of Eligibility Under Special Laws: PHP500.00. 

NOTE: This DOES NOT include issuance of COE. If you wish to be issued a COE after your application has been approved, see Issuance of Original COE and Issuance of Authenticated Copy below.

 

2. Issuance of Original COE: PHP100.00/copy (maximum of 3 copies only). For multiple requests for original COE, provide us with the commensurate number of 1x1 photos.

3. Issuance of Authenticated Copy of the COE: PHP50.00/copy

 

4. Correction of Personal Information [COPI]: PHP760.00

 

5. Issuance of Certificate of Discrepancy: PHP50.00/copy 

 

6. Issuance of Eligibility Card (for May - December 2015 Passers only): PHP200

NOTE: The Eligibility Card may be only released at the CSC Regional Office in Legazpi City, and CANNOT BE FORWARDED TO THE CSC FIELD OFFICES. 

 

 

PAYMENT DETAILS

 

For your protection, our main mode of payment for transaction fees shall be via POSTAL MONEY ORDER (PMO). Make the PMO payable to CIVIL SERVICE COMMISSION REGIONAL OFFICE V.  Requests received without enclosed payment, or PMOs with an incorrect payee will NOT be processed. These shall be returned to the client for compliance.

The PMO is a measure we have come up for the safety of your payment, as only the payee indicated therein can encash the same. If you choose to enclose cash inside your envelope, and your courier accepts delivery of your parcel, then we shall accept your documents. However, CSC RO V will NOT BE RESPONSIBLE FOR THE POSSIBLE LOSS OF YOUR CASH IN TRANSIT.
