RETURN AND RELEASE OF DOCUMENTS

RELEASE OF DOCUMENTS FOR FIRST TIME TRANSACTIONS*:


Once your documents have been processed, if this is your first-time transaction (first time to claim a COE, or those newly-approved application for Special Eligibilities) you shall be required to PERSONALLY APPEAR at the CSC Regional Office in Legazpi City, or the CSC Field Office in your respective province (outside of Albay within Bicol) to claim your documents.

NOTE THAT THESE WILL NOT BE RELEASED TO AN AUTHORIZED REPRESENTATIVE. 

You are required to sign pertinent documents relative to the release of your eligibility.  

If you are based outside of Albay, COMPLETE THE "RETURN OF DOCUMENTS" PORTION IN THE ERRF and indicate the CSC FO within Bicol where you will personally claim your document.

 

IMPORTANT NOTICE!

Please be informed that we shall send your documents to the CSC FO by batch on a weekly basis. If you want your documents to be arrive earlier, YOU MAY PROVIDE A PRE-PAID COURIER PACK ADDRESSED TO THE CSC FIELD OFFICE WITHIN BICOL REGION WHERE YOU WOULD LIKE TO PICKUP YOUR DOCUMENTS.

Please wait for our officer in the FO to contact you with regards to the availability of your documents before proceeding to the. As such, ensure that you provide an active mobile number in your ERRF.

 

*The term "first time transaction" is on a PER ELIGIBILITY basis. If you have multiple eligibilities, say Professional and Subprofessional, even if you have already claimed your COE for the Subprofessional eligibility, but this is your first time to claim your Professional COE, you are still considered under first time transaction for the said eligibility. Also, first time transactions also cover those who underwent processing of Correction of Personal Information, if this is also their first time to claim the COE for the specific eligibility.


RETURN/RELEASE OF DOCUMENTS FOR NON FIRST TIME TRANSACTIONS:

For non-first time transactions (request for issuance of authenticated copy of the COE, or the re-issuance of a lost / damaged COE), you have two options for the release of your documents:

1. Pick-up of documents at the CSC Regional Office by you or an authorized representative [Provide an original signed authorization letter to your representative. Also, your representative must have a valid ID]; and

2. Pick-up at the nearest CSC Field Office (FO) within Bicol by you or an authorized representative [Provide an original signed authorization letter to your representative. Also, your representative must have a valid ID].
