ALTPROC: Status of Request / Application

Once you have sent your documentary requirements via mail or courier, or have submitted it through the RO / FO Dropbox, allow lead time for your documents to reach us. There are delays with the courier / Post Office due to the current situation. Further, as part of our strict health and safety protocols, all documents (including those received via the RO DropBox) are QUARANTINED FOR AT LEAST THREE DAYS before they are handled by our processors.

 

ONLINE IDENTITY VERIFICATION

We shall require online identity verification via Facebook Messenger or Zoom video call for those:

1. Non-first time requests who wish to have their documents mailed directly to them via a self-addressed, stamped envelope or a pre-paid courier parcel; or

2. First time requests (including newly-approved SPELS applications) who wish to claim their documents at the CSC RO V Field Office (FO) outside of Albay.

In line with this, you are advised to provide us with your Facebook handle (e.g. www.facebook.com/esd.rov.9) in the ERRF , or we shall send you a link for a Zoom meeting for those who would like to opt for Zoom, so that we may conduct a video call to you. Please note that processing of your request will not proceed if you cannot undergo this online verification.

To follow-up / check the status of your request / application, click the appropriate button below. Please note that if your name is not yet listed therein, either we have not yet received your documents, or they are still under quarantine, as stated above.

RELEASE OF DOCUMENTS FOR FIRST TIME REQUESTS (INCLUDING NEWLY-APPROVED APPLICATION FOR SPELS)

Our policy on the release of eligibility for first time requests state that you are required to PERSONALLY claim your documents at the CSC Regional Office in Rawis, Legazpi City or in the CSC RO V FO in your province (based on the current address you have indicated in the ERRF / application form), and sign pertinent documents relative to the release of your eligibility.

Please note, however, that we DO NOT RELEASE DOCUMENTS TO CSC RO and FO outside of Region V.
