ALTPROC: Requirements for Issuance of COE

Eligibles who have taken their exam or have been conferred an eligibility in Region V with unclaimed / lost  / damaged / corrected eligibility may request for a Certification of Eligibility in security paper (COE), which will serve as an original proof of having an eligibility

Prepare the following documentary requirements to be issued a COE:


1.) Duly accomplished Exam/Eligibility Records Request Form (ERRF) and Declaration Form (NOTE: Please use OUR VERSION of the ERRF and Declaration Form, downloadable HERE). 

 

Download and print the ERRF and Declaration Form, and properly fill-out the same. Ensure that all necessary data (personal information such as full name, date of birth and place of birth, contact number, and examination details such as title of exam/eligibility, place of exam and date of exam or issuance) are provided. Do not forget to affix your right thumb mark in the box provided both in the ERRF and Declaration Form, and affix your signature therein.

 

For those with multiple eligibilities, YOU SHALL BE REQUIRED TO FILL-OUT ONE ERRF and DECLARATION FORM PER ELIGIBILITY requested.

2.) One (1) ORIGINAL valid government-issued ID. The ID should be valid (i.e., NOT EXPIRED) upon submission. It should also contain your FULL NAME, DATE OF BIRTH (DOB), and a CLEAR PHOTO of you, and must be signed both by the bearer and the issuing authority.

 

NOTE: To reiterate, we require the ORIGINAL ID, not a photocopy, while your request is being processed. If you are using the ID you have presented during the pre-evaluation (esp. driver's license, company ID), be advised to provide us with another ID that you can send/submit to us while we are processing your request.

3.) PSA-issued marriage certificate (for married female eligibles only). The marriage certificate shall only be required for those who have taken the exam or have been conferred an eligibility when they were still single, and will now present an ID bearing their married name.


4.) 1” x 1” photo, with nametag and signature. The photo must be in white background, in standard close-up format (the face must cover 80% of the entire photo), and taken within the last three (3) months. The eligible must hold a HANDWRITTEN nametag with signature above the name, placed no more than 1 inch under the chin prior to having the photo taken. Accessories and / or any eye wear should be removed. Name in the tag should be in the First Name, MI, Last Name, Suffix (if any) format. Your face and nametag must be clear in the photo (not too dark and blurred), and it should be printed in quality photo paper.
 
 
5. ) Transaction Fee: Certification fee is P100.00 / copy. You may be issued only up to three (3) copies of the COE. For multiple copies, provide us with the commensurate number of 1” x 1” photos. Indicate the number of copies requested in the appropriate box in the ERRF.

You may also include in your request the issuance of AUTHENTICATED COPIES of your COE (this is a requirement for employment and promotion in the government). Authentication fee is P50.00 / copy. Indicate the number of copies requested in the appropriate box in the ERRF. You do not need to fill-out another ERRF for this.


The COE security paper and its authenticated copies have NO EXPIRY.
