ALTPROC: Requirements for Issuance of Authenticated Copies of the COE

Eligibles who have taken their exam or have been conferred an eligibility in Region V who currently hold an original COE may request for certified authenticated copy/ies of the same. This is a requirement for employment or promotion in the government.

Prepare the following documentary requirements to be issued authenticated copies of your COE:

 

1. ) Original Certificate of Eligibility(COE): For those who have not yet requested for an original COE, or if your original COE was lost or damaged, you must request for its issuance BEFORE YOU MAY BE ISSUED AN AUTHENTICATED COPY THEREOF. To request for an original COE, see Issuance of COE link below.
 
For those who still have their original COE (including the one printed in yellow security paper), the same must be in good condition. Any physical damage that renders any part of the certificate non-legible, or any modification made therein (including replacement of or damage to the attached picture) shall invalidate your COE. You shall be required to request for the replacement thereof (see Issuance of COE).

 



IMPORTANT NOTICE TO PROFESSIONAL BOARD EXAM FOR TEACHERS (PBET) PASSERS



PLEASE NOTE THAT THIS IS ONLY AN INVITATION SLIP TO ATTEND THE AWARDING CEREMONY, AND DOES NOT SERVE AS AN ORIGINAL COE. As such, we do not provide an authenticated copy of this document. If you only have this slip as proof of passing the PBET, then you shall also be required to request for the Issuance of COE (see link above).


2.) Duly accomplished Exam/Eligibility Records Request Form (NOTE: Please use OUR VERSION of the ERRF, downloadable HERE).  

 

Download and print the ERRF, and properly fill-out the same. Ensure that all necessary data (personal information such as full name, date of birth and place of birth, contact number, and examination details such as title of exam/eligibility, place of exam and date of exam or issuance) are provided. Do not forget to affix your right thumb mark in the box provided, and your signature. For authentication requests, you do not need to fill-out the Declaration Form. 

 

NOTE: For those with multiple eligibilities, YOU SHALL BE REQUIRED TO FILL-OUT ONE ERRF PER ELIGIBILITY requested.

3.) One (1) ORIGINAL valid government-issued ID. The ID should be valid (i.e., NOT EXPIRED) upon submission. It should also contain your FULL NAME, DATE OF BIRTH (DOB), and a CLEAR PHOTO of you, and must be signed both by the bearer and the issuing authority.

 

NOTE: To reiterate, we require the ORIGINAL ID, not a photocopy, while your request is being processed. If you are using the ID you have presented during the pre-evaluation (esp. driver's license, company ID), be advised to provide us with another ID that you can send/submit to us while we are processing your request.

4.) PSA-issued marriage certificate (for married female eligibles only). The marriage certificate shall only be required for those who have taken the exam or have been conferred an eligibility when they were still single, and will now present an ID bearing their married name.




5. ) Transaction Fee: Authentication fee is P50.00 / copy. Indicate the number of copies requested in the appropriate box in the ERRF.

The COE security paper and its authenticated copies have NO EXPIRY.
